Job Description:
(Physician/MD qualifications required) Physician - The crisis
clinician provides crisis intervention services within the home,
office and community setting to youth, adults and families
experiencing a behavioral health crisis. This licensed provider
will engage as a member of a collaborative team. Using trauma
informed approach and representing the organization on how we offer
integrated services for recovery and mental health. Must provide
phone and community-based crisis responses to youth and adults
presenting with urgent mental health needs.
Services include:
Answering the crisis hotline.
Providing resource information and linkage to community
providers.
Complete crisis assessments in community or hospital settings; and
safety-disposition planning.
Deliver positive results as evidenced by utilization of outcome
measures.
Perform direct service at 100% of billable expectations.
De-escalate-stabilize targeted clients.
Link clients to appropriate level of care and services including
peer support, assuring safety, develop crisis plan, provide
information as appropriate to family-significant others and
resolving the eminent situation.
Provide and complete risk-crisis assessments of individuals
regarding level of danger to self or others, severity of
psychiatric symptoms, and existing support system to determine a
recommendation of continued care; outside hours will be
required.
Short term case management and crisis counseling services as
needed.
Conduct follow up visits with consumers recently discharged from
inpatient hospitals to provide linkage to appropriate outpatient
services.
Coordinate services with consumers CPST worker and prescriber.
Coordinate services with emergency transport and local hospitals
when necessary.
Attend and initiate interdisciplinary case conferences both intra
and interagency.On-site clinical supervision presence 24-7; great

Education-Experience:
Bachelors Degree in Social Work-Counseling Required
Masters Degree in Social Work- Counseling (preferred)
Two (2) years of experience in the mental health field
(preferred)
Licensure:
LPC-LSW Required
Physical Requirements:
Drug Screen, TB Test, Background check (required)
Work in the community with frequent travel to client homes and
other community locations
Must be comfortable making home visits throughout the community
Full time hours; 4-10-hour shifts, 3-12-hour shifts, 5-8-hour
shifts
Ability to work at various locations as needed
Ability to work evenings, overnight shifts and weekends as
needed
Ability to participate in events off-site
Ability to frequently sit in a stationary position for extended
period of time
Ability to frequently use hands-fingers to type on computer
keyboard and operate general office equipment, such as phone and
copier
Ability to occasionally-frequently ascend- descend stairs as
necessary
Ability to frequently drive
