Job Description:

Counselor/Therapist - The Counselor, Community Based, offers
diagnostic assessment and counseling within the home and community
setting to youth and families with increased risk factors,
multi-system involvement, and-or needing a higher level of care
than Outpatient. This position is responsible for individual,
family, and group services. This licensed provider will engage as a
member of a collaborative team. With a Trauma Informed approach,
this position will be a representative for the organization and how
we offer integrated services for recovery and mental health.
Positions may be specific to general population, intensive
services, or a combination of both.
Level 1: Bachelors level LSW
Level 2: Masters level LPC-LSW-MFT
Essential Duties
Provide trauma informed services to clients and their family
system. Services are provided primarily in the home and community.
Primary service is family therapy, with some crisis response,
individual therapy, diagnostic assessments and group therapy. This
position incorporates Evidence Based treatment modalities. Deliver
positive results as evidenced by utilization of outcome measures.
Perform direct service at 100% of billable expectations. 55% of
your time will be on direct client care.
Provide accurate, concise, and concurrent documentation according
to program procedures and policies. Including, but not limited to
Diagnostic Assessments, Treatment Plans, and Progress Notes. 20% of
your time will be on Documentation.
Provide consultation and collaboration with community partners,
specifically Job and Family Services, Juvenile Court, and schools.
Promote Crossroads Health services and engage in outreach
activities. You provide 15% of your time on relationship
building.
Seek and secure professional development consistent with licensure
and credential requirements for personal and organizational growth.
It requires at least 5% of your time on professional
development.
Capitalize on opportunities to support programmatic needs across
the organization
o Participate in supervision and team meetings, work groups, task
forces, or other general efforts that add value to the success of
the organization and programming
o Provide crisis intervention-support as needed as part of a 24-7
On Call rotation o Other duties as needed . 5% of your time on
Miscellaneous programmatic duties and other organizational
support.
Experience
At least one (1) year of experience in the mental health field
required for Bachelors level LSW
Two (2) years of experience in the mental health field
(preferred)
Education
Bachelors Degree in Social work with an LSW will be considered
Masters Degree in Social Work-Mental Health (preferred)
Licensure-Certifications
Licensed in the State of Ohio
o LPC-LSW-MFT (required)
Must have an acceptable driving record (4 points or less), reliable
transportation, and carry automobile insurance with minimum
liability limits of $100,000-$300,000 and property damage of
$50,000.
BLS-First Aid (as applicable, company provided)
