We currently have the following opportunity for an energetic, skilled, and compassionate individual like you to become part of the Lake Health team.Position Summary:The RN, under the direction of the Clinical Manager is responsible to the Director, Lake Health Home Care Services, and its clients for delivering care to patients utilizing the nursing process of assessment, planning, intervention, implementation and evaluation. The RN effectively interacts with patients, significant others and other health team members while maintaining standards of professional nursing.Essential Job Functions:Demonstrate utilization of the nursing process through appropriate assessment, planning, implementation and evaluation of the patient's care.Communicate and collaborate with physicians, additional disciplines involved in the patient's care, along with the patient and family members to ensure development and implementation of an appropriate plan of care.Evidenced by documentation review, results of SHP review, and submission data.Demonstrates support of an environment conducive to patient privacy, safety and satisfaction.Actively participates and contributes to agency performance improvement initiatives and improvement of patient outcomes.Direct home care aides and assign appropriate duties by supervising their work and instructing on proper performance of tasks assigned.Adheres to Lake Health System and agency policies and procedures, mission, vision and values, along with ability to adapt to changes in the healthcare environment.Other duties as assigned.Education and Experience:Associate degree required. BSN preferred.License/Certification: Valid license as a Registered Nurse in the state of Ohio.
