Job Description:
Job Description Description Community Outreach Mission &

Outreach role focused on expanding HHVI’s mission throughout the
community and reaching populations we have not previously served.
The position supports existing and/or future outreach efforts with
UH while also independently planning, organizing, and executing

appropriate referrals and follow-up care. The role typically

participating in large-scale community partnerships (e.g., UH
Health & Safety Expo, Cleveland Browns events, and other regional
activities). This position requires strong initiative and
leadership—someone who can “take the bull by the horns,” manage
outreach logistics independently, and collaborate closely with the
Director to align activities with organizational goals. The role
