Job Description Job Description The Home Health Physical Therapist provides skilled physical therapy services to patients in their homes to help restore mobility, improve functional independence, reduce pain, and prevent disability. The therapist works as part of an interdisciplinary home health team to develop and implement individualized treatment plans that support patient safety and quality of life. Compensation: $55 - $90 hourly Responsibilities: Evaluate patients’ physical condition and functional limitations in the home setting Develop and implement individualized physical therapy treatment plans Provide therapeutic exercises, manual therapy, gait training, balance training, and mobility instruction Educate patients and caregivers on safe mobility, fall prevention, and home exercise programs Assess home environments and recommend adaptive equipment or modifications Monitor patient progress and adjust treatment plans as needed Document all evaluations, treatments, and patient progress accurately and in a timely manner Communicate regularly with physicians, nurses, and other healthcare team members Ensure compliance with home health regulations and agency policies Participate in case conferences and care coordination activities Qualifications: Education: Currently licensed Experience: Two years of experience in Home Health is preferred Skills: Able to maintain strict confidentiality with regard to protected and sensitive information Able to establish and maintain the confidence of owners and employees Must be able to solve problems independently and confidently by applying analytical and logical thinking Able to use standard office equipment and software proficiently Language Skills: Able to read, analyze, and interpret reports, general business directives, policy and procedure statements, and governmental regulations. Able to write reports and correspondence at a professional level Able to immediately respond to common inquiries and complaints from employees and regulatory agencies Able to effectively communicate with individuals from diverse backgrounds and varying educational levels Able to effectively organize and present information and respond to questions from employees and external groups of varying sizes About Company At Home Care Wellness , we know that quality patient care starts with supporting our team.
