Job Description:
$2,000 Sign-On Bonus Crossroads Health provides a continuum of
quality life-changing behavioral health and specialized chemical
dependency treatment services to children, adolescents, young
adults, adults and families. As a leader in the behavioral health

evaluate, educate, strengthen and support thousands of children,
adults and their families each year. Programmatic expansion is
providing our School Based Services to add the role of School Based
Case Manager. The School Based Case Manager has the unique
opportunity to work with clients in the school setting. Based in a
school, this role works independently to develop relationships with
clients, families, school officials and other interested parties
with meeting the students' mental and behavioral health needs. This
role will also provide assessments, individual, group and family
therapy as needed while also acting as a strong advocate for
clients and consultant with our school partners. An
interdisciplinary approach offers the opportunity to collaborate
with a diverse group of colleagues. This position comes with a
$2,000 sign on bonus. Primary Responsibilities: Provide mental and
behavioral health services to children and adolescents in Lake
County school systems. Consult with parents, gatekeepers, teachers
and others in the community. Provide group and individual services
in the schools. Provide links between parents and community
resources. Provide crisis intervention as needed. Evenings as
dictated by client and district needs. Home visits as dictated by
client and district needs. Requirements: 1 years of experience with
children/adolescents mental health assistance. Bachelor's degree
required, Master's degree preferred in Counseling, Social Work or
other related fields Willingness to attain QMHS upon hire required
without licensure. Demonstrated experience building relationships
with children and adolescents. Excellent communication skills -
particularly verbal and written communication. Reliable
transportation. Must have valid driver's license (4 points or less)
and carry minimum auto liability insurance of $100,000/$300,000 and
